Rabies Serum Neutralization Assay Requisition

NOTE:  Specimen MUST be refrigerated as soon as possible.  

NOTE:  Specimen MUST be aliquoted into a 2 ml Sarstedt microcentrifuge tube.

NOTE:  Requisition must be completed by the ordering physician including occupational category, vaccination history, administration of RIG, animal exposure and travel history.

NOTE:  Individuals who have received pre- or post-exposure vaccination for rabies virus.  The Rabies, Rickettsia and Related Zoonotic Diseases laboratory must be notified prior to submission of any samples for clinical investigation.
